Unit Case Management/Care Coordination
# patients seen/consults per day 15-20
Certs: BLS
EMR: Interqual
Weekend Requirement every other depending on staffing. Would be at least one full weekend per month
Holidays not required, but will work if they are willing
Scrub Color/Uniform business causal attire
Hospitalist or Intensivist in house yes
RT 24/7 yes
Pharmacy 24/7 no
Shifts depends on dept but hours are between 8-5pm
Do they cover all areas of hospital? Any outpatient? Any home care? All areas of the hospital, no OP and no Home Care
Floor specific orientation: 5 days of care coordination new employee orientation, unit orientation timeframe depends case by case
Common duties/ Required skills or knowledge: Dealing with all payers, uninsured, homeless. Completing Initial assessments, IQ, DC planning, cerner
