Work Environment

The FNP will work onsite at our outpatient office, which is designed to foster patient comfort and staff collaboration. Key Responsibilities

• Conduct thorough health assessments and physical examinations for patients across all age groups.
• Order and interpret diagnostic tests, using the results to inform patient management and treatment decisions.
• Develop patient-centered care plans in collaboration with team members, ensuring they align with the latest clinical guidelines and best practices.
• Prescribe medications, based on evidence-based practices, while monitoring patient responses and adjusting treatments as necessary.
• Provide detailed health education and counseling to patients and their families, equipping them with the knowledge necessary to manage their health.
• Document patient encounters meticulously within the EHR system, maintaining accurate and up-to-date medical records.
• Participate in team meetings and interdisciplinary case conferences, actively discussing patient progress and care strategies.
• Engage in quality improvement initiatives to enhance patient outcomes and optimize clinical performance.
• Maintain compliance with state and federal regulations related to healthcare practices.

Patient Population / Client Focus

In this role, the FNP will cater to a diverse range of patients, from newborns to older adults. This includes managing routine check-ups, vaccinations, and preventive screenings, as well as providing care for acute illnesses and chronic conditions such as diabetes and hypertension. Qualifications

• A valid and unrestricted Nurse Practitioner license in New York.
• A Bachelor’s Degree in Nursing, with a focus on Family Nurse Practitioner training preferred.
• A minimum of 2 years of experience in a Family Nurse Practitioner role, preferably in an outpatient setting.
• Strong understanding of clinical practices, procedures, and protocols in a primary care environment.
• Exceptional clinical assessment abilities and diagnostic skills.

Compensation and Benefits
