Work Environment

This role is based in an outpatient office setting, where you will work directly with patients in a supportive environment. The office operates primarily on weekdays, with a schedule from 9:00 a.m. to 6:00 p.m., including some evening clinics until 8:00 p.m. Participation in community outreach events and health fairs on weekends may be necessary, reflecting our commitment to serving the community beyond regular office hours. The average workweek totals around 40 hours, balancing clinical responsibilities with community engagement.

Key Responsibilities

• Conduct thorough health assessments and screenings tailored to the unique needs of individuals and families, ensuring a full understanding of their medical histories and present conditions.
• Diagnose and manage a variety of acute and chronic health conditions, utilizing evidence-based practices to promote preventive care and health education.
• Work collaboratively with community organizations, developing and implementing health education programs aimed at raising awareness about important health issues.
• Provide culturally sensitive care by understanding and respecting the diverse backgrounds of our patients, advocating actively for their healthcare rights.
• Coordinate referrals and ensure comprehensive follow-up care for patients, maintaining continuity of care and addressing any barriers to access.
• Maintain clear and accurate medical records that comply with health regulations, integrating modern electronic health record systems for effective data management.
• Engage in community outreach initiatives, participating in health fairs and other events to provide health screenings and promote overall wellness in the community.
• Stay informed on current community health trends, policies, and best practices, contributing to ongoing service improvement and quality assurance efforts.

As a Family Nurse Practitioner at Little Vista Health Group, you will serve a diverse population, including families, children, and the elderly. Qualifications

• Must hold a current certification as a Family Nurse Practitioner.
• Possession of an active nursing license in the state of New York.
• A Master’s degree in nursing from an accredited institution is required.
• Minimum of 3 years of clinical experience, with a preference for backgrounds in community health or public health settings.
• Demonstrated strong interpersonal and communication skills, enabling effective collaboration with diverse populations.

Preferred Experience

Compensation and Benefits
