Jackson Medical Center is actively seeking a dedicated and experienced Family Nurse Practitioner to join our dynamic healthcare team in Windsor, New York. Work Environment

This role is based in our outpatient office in Windsor, ensuring direct contact with patients in a supportive and collaborative setting. As a Nurse Practitioner, your typical work schedule will be Monday to Friday, from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending to 8:00 p.m. Additionally, you may participate in community outreach events or health fairs on weekends, contributing to initiatives designed to improve health literacy and access.

Key Responsibilities

• Conduct thorough health assessments and screenings for a diverse array of patients, managing both acute and chronic conditions.
• Develop individualized care plans that encompass preventive care initiatives and health promotion strategies tailored to specific community needs.
• Collaborate closely with interdisciplinary teams and community organizations to develop and execute educational programs that address prevalent health issues.
• Provide culturally competent care that respects the beliefs and values of all patients while advocating for their access to healthcare services.
• Ensure continuity of care through effective coordination of referrals and comprehensive follow-up with patients and specialists.
• Maintain accurate and detailed electronic medical records in compliance with healthcare regulations and organizational standards.
• Engage actively in community events to promote health awareness, provide health screenings, and foster direct community engagement.
• Stay informed on current public health trends and evidence-based practices to enhance service delivery and patient outcomes.

You will primarily serve a diverse patient population, including families and individuals from various socioeconomic backgrounds in Windsor and its surrounding areas. Qualifications

• Current certification as a Nurse Practitioner and active state nursing license are required.
• A Master’s degree in nursing from an accredited program is mandatory.
• A minimum of four years of clinical experience, ideally in community health or public health settings, demonstrating a strong understanding of diverse community needs.
• Exceptional communication and interpersonal skills, with the capability to work effectively with varied populations.

Preferred Experience

Compensation and Benefits
