Work Environment

This role will be performed onsite at our outpatient office, where you will engage in a dynamic and supportive work setting. The expected work schedule ranges from 9:00 a.m. to 6:00 p.m., occasionally extending to 8:00 p.m. for evening clinics, with an average workweek of approximately 40 hours. Participation in various community outreach events and health fairs during weekends will be part of your responsibilities, allowing you to extend care beyond the clinic walls and into the heart of the community.

Key Responsibilities

• Conduct thorough health assessments for patients of all ages, including routine screenings and evaluations that consider both physical and emotional health.
• Diagnose and effectively manage a wide range of acute and chronic medical conditions, utilizing a patient-centered approach that prioritizes preventive care and health education.
• Collaborate closely with local organizations and stakeholders to develop and implement community health education programs designed to inform and empower individuals regarding their health.
• Advocate vigorously for the rights of patients, ensuring equitable access to healthcare services, covered treatments, and preventive care resources.
• Coordinate patient referrals and follow-up care to promote continuity and quality of care across various healthcare settings, improving overall patient experience.
• Maintain detailed and accurate medical records, adhering strictly to regulatory guidelines and ensuring confidentiality and compliance.
• Participate actively in community outreach initiatives to engage the public in health screenings and wellness programs, demonstrating commitment to public health.
• Stay informed of community health trends, evidence-based practices, and regulatory changes that impact patient care and community health strategies.

Patient Population / Client Focus

Qualifications

Preferred Experience

Compensation and Benefits
