Key Responsibilities

As a Nurse Practitioner at Uprise Valley Pediatrics, your primary duties will include:

• Conducting thorough health assessments and screenings to gather comprehensive data about patients' health status.
• Diagnosing and managing both acute and chronic medical conditions while promoting preventive care and health education.
• Collaborating with local organizations to develop educational programs tailored to community health needs, enhancing awareness about common health issues.
• Delivering culturally sensitive care that respects diverse backgrounds and advocates for patients' rights and access to services.
• Coordinating referrals and follow-up care to ensure a seamless transition through different healthcare services, maintaining continuity for our patients.
• Maintaining accurate and up-to-date medical records in adherence to healthcare regulations, ensuring confidentiality and compliance.
• Participating actively in community outreach events and health fairs, delivering screenings and health education to underserved populations.
• Staying current with trends in community health, evidence-based practices, and legislative impacts on healthcare delivery.

Patient Population / Client Focus

This role focuses on providing care to a diverse patient population, including children, adolescents, and families within the community. You will engage with underserved populations, promoting health equity and bridging gaps in access to healthcare services. Your expertise will not only address immediate health concerns but also empower patients with the knowledge and resources to maintain their health independently.

Qualifications

The ideal candidate will possess the following qualifications:

• Current certification as a Nurse Practitioner.
• An active state nursing license in New York.
• A bachelor's degree in nursing from an accredited institution; a master’s degree is preferred.
• Strong interpersonal skills, demonstrating the ability to communicate effectively and empathetically with patients from various backgrounds.

Preferred Experience

• A minimum of 3-5 years of clinical experience, particularly in community health or public health practices.

Compensation and Benefits

Uprise Valley Pediatrics offers a competitive salary range of $120,000–$220,000 annually, commensurate with experience and qualifications. In addition to competitive pay, we provide a comprehensive benefits package that includes:
