This role is set within a dynamic outpatient office environment where you will work alongside a multidisciplinary team, including physicians, nurse practitioners, physician assistants, medical assistants, and care managers. Key Responsibilities

• Provide exceptional primary care services to adult patients, conducting both routine and acute office visits.
• Administer annual wellness visits and health risk assessments, emphasizing detailed and accurate documentation and a holistic approach to health.
• Coordinate care with various providers, specialists, and community agencies to ensure comprehensive patient care.
• Lead population health initiatives, such as ensuring eligible patients receive evidence-based screenings, including breast cancer screenings every two years.
• Assist the care team with phone triage and patient outreach, enhancing overall communication and support.
• Educate patients on their medical conditions, treatment plans, and health maintenance strategies to empower them in their health journeys.
• Engage in community outreach and promotional activities to raise awareness about our services.
• Conduct home visits as needed to support patients in their own environments.
• Report directly to the Medical Director and collaborate closely with operational leadership.

Patient Population / Client Focus

This role primarily involves working with adult patients, particularly those aged 65 and older, who are covered by Medicare. Our healthcare model is built around creating personalized care plans that cater to the unique needs of older adults. We emphasize preventive health care to help our patients maintain a high quality of life while managing chronic conditions effectively.

Qualifications

• Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or successful completion of an NP Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• An active, non-probationary state Nurse Practitioner license and valid DEA license.
• Demonstrated expertise in primary care, particularly within internal medicine, geriatrics, or family medicine.
• Proficient in HCC documentation, ICD-10 Coding, and conducting Health Risk Assessments.

Preferred Experience

• A passion for teamwork and a collaborative approach to patient care.
• Interest in innovative health models focused on population health management.
• Bilingual proficiency in relevant languages is a significant advantage.

Compensation and Benefits

The salary range for this position is competitive, between USD 115,000 to 150,000 annually, depending on experience and qualifications. We offer a comprehensive benefits package that includes:
