We are seeking a dedicated and skilled Internal Medicine Nurse Practitioner to join our team at Northwell Health in Lancaster, New York. This full-time position will focus on providing comprehensive home care services to patients with various internal medicine conditions. Work Environment

This role is primarily onsite, providing home care services in the Lancaster area, allowing you to develop close, personal relationships with patients in their own environments. The collaborative work setting includes interactions with healthcare professionals, such as attending physicians, nursing staff, and other members of the care team, creating a supportive environment which encourages professional development and teamwork. You will utilize the latest telehealth technologies, electronic health records, and other medical tools to facilitate patient care.

Key Responsibilities

• Conduct comprehensive assessments by taking detailed patient histories and performing thorough physical examinations to inform diagnosis and treatment plans.
• Develop, document, and communicate individualized plans of care, ensuring thorough screening and preparation for patients before treatment.
• Carry out daily patient rounds and provide regular updates to the medical team regarding patient progress, needs, and outcomes.
• Write and manage orders for medication, laboratory tests, and diagnostic imaging, carefully interpreting laboratory and test results to adjust treatment plans as necessary.
• Engage in multidisciplinary team meetings to discuss complex cases, coordinate care, and strategize improvements in the care delivery process.
• Document discharge summaries upon patient discharge, including prescriptions and referrals, and ensure patients understand their post-discharge care instructions.
• Participate actively in performance improvement initiatives, quality assurance programs, and ongoing research to enhance healthcare delivery practices.
• Maintain compliance with state regulations and organizational policies in all professional activities, fostering a culture of safety and accountability.

Patient Population / Client Focus

This position primarily focuses on adult patients with a range of chronic and acute internal medicine conditions. You will work closely with patients who require ongoing management of their health issues, including but not limited to diabetes, hypertension, chronic obstructive pulmonary disease (COPD), and heart disease. By providing personalized care in a home setting, you will have the opportunity to make a significant impact on enhancing patients' quality of life and managing their health conditions effectively.

Qualifications

• A Master’s Degree in a Nurse Practitioner Program is required, along with National Board Certification in a relevant specialty area.
• Current licensure to practice as both a Nurse Practitioner and Registered Professional Nurse in New York State is necessary.
• A collaborative practice agreement with established protocols filed with the New York State Department of Education or a collaborative relationship attestation as per the Nurse Practitioner Modernization Act 2015 must be in place.
• Current Basic Life Support (BLS) certification is required to ensure patient safety and care standards.

Preferred Experience

While not mandatory, previous experience in an internal medicine setting or home care is highly preferred. Candidates with proven skills in patient assessment, care coordination, and the ability to work in a fast-paced environment will be given priority. Familiarity with electronic health record systems and telehealth practices will also be advantageous in this role.

Compensation and Benefits

This full-time position offers a competitive salary ranging from USD 150,000 to 160,000 per year, commensurate with experience and qualifications.
