Work Environment

This role is onsite, enabling the Nurse Practitioner to provide hands-on care directly in the community. Our work setting emphasizes concierge medicine, allowing for an intimate practitioner-patient relationship enhanced by house calls. Key Responsibilities

As an Internal Medicine Nurse Practitioner, you will take on a range of essential responsibilities designed to meet the needs of our patient population:

• Conduct comprehensive health assessments, including detailed medical histories and physical examinations, documenting findings accurately.
• Diagnose and manage acute and chronic health conditions, offering personalized care plans tailored to individual patient needs.
• Order and interpret diagnostic tests such as laboratory studies and imaging, facilitating informed clinical decisions.
• Prescribe both pharmacologic and non-pharmacologic treatments, ensuring a holistic approach to wellness and recovery.
• Set patient-specific outcomes and regularly assess and update treatment plans based on efficacy and patient feedback.
• Engage patients in health promotion activities, providing education on disease prevention and management, contributing to better health outcomes.
• Collaborate with other medical professionals within a multi-specialty framework to optimize patient care.

Patient Population / Client Focus

Your primary focus will be on adult patients requiring comprehensive internal medicine care. This includes managing chronic diseases prevalent in this demographic, such as diabetes, hypertension, and heart disease. The internal medicine specialty is dedicated to treating complex conditions and coordinating care across multiple healthcare providers to enhance health outcomes.

Qualifications

To be considered for this position, candidates must meet the following qualifications:

• A Master's degree in Nursing with specialization as a Nurse Practitioner.
• Current licenses as a Registered Professional Nurse and Nurse Practitioner in the State of New York.
• Board Certification as a Nurse Practitioner.
• Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications are required.
• Exceptional clinical skills coupled with strong interpersonal and communication abilities are paramount.

Preferred Experience

Compensation and Benefits
