As a Nurse Practitioner within our Home Care facility, you will work in a supportive and dynamic environment, collaborating with a dedicated team of healthcare professionals. Key Responsibilities

• Deliver comprehensive and high-quality care in accordance with established national and community standards for internal medicine.
• Work collaboratively with office staff to develop and enhance efficient operational practices, ensuring optimal patient care.
• Serve as a liaison between practice management and hospital administration, effectively communicating the needs and concerns of both parties.

• Ensure compliance with all regulatory guidelines affecting patient care and institutional operations.
• Manage patient care effectively while aligning with both health system goals and practice objectives.

Patient Population / Client Focus

The Nurse Practitioner will primarily focus on adult patients within the home care context, addressing a wide array of chronic and acute health conditions. This role requires not only clinical expertise in outpatient clinical medicine but also the ability to interpret, assess, and act upon patient data that reflects their individual needs, particularly tailored to age-specific requirements.

Qualifications

• Current NYS certification/registration as a Family, Adult, or Pediatric Nurse Practitioner.
• AANP Board Certification is mandatory.
• A valid DEA license is required for this position.
• Ability to obtain the appropriate privileges to practice at F. F. Thompson Health.
• Preferably possess familiarity with ICD10 coding for optimal patient record management.

Preferred Experience

• Prior experience in a Nurse Practitioner role is a significant advantage, particularly in a home health or outpatient setting.
