Work Environment

Located in the heart of New York City, our assisted living facility provides a supportive and collaborative environment. As a Nurse Practitioner, you will work onsite, engaging with an interdisciplinary team of healthcare professionals to ensure optimal patient care. The work schedule typically spans from 9:00 a.m. to 6:00 p.m., with occasional evenings extended until 8:00 p.m., and may include participation in community outreach initiatives, such as health fairs on weekends.

Key Responsibilities

• Perform thorough health assessments and screenings of residents, evaluating both acute and chronic conditions.
• Formulate and implement individualized care plans that include preventive measures and health promotion strategies.
• Collaborate with physicians, nurses, and other healthcare practitioners to ensure comprehensive management of resident health.
• Advocate for patients' rights, providing culturally sensitive approaches to healthcare that respect each patient's background and preferences.
• Coordinate referrals and ensure seamless continuity of care through diligent follow-up and communication with other healthcare providers.
• Maintain meticulous and compliant medical records, ensuring adherence to health regulations and ethical standards.
• Engage actively in community health initiatives, convening workshops and health education programs designed to empower residents and their families.
• Stay informed about relevant health trends, policy changes, and best practices, enhancing both personal and organizational performance.

Patient Population / Client Focus

This role primarily involves working with elderly residents of the assisted living facility, many of whom present with complex medical histories and multifactorial health needs. You will be instrumental in ensuring that their physical, emotional, and psychological needs are addressed with respect and compassion. Your care will not only impact individual health outcomes but also contribute to the overall wellness of the community.

Qualifications

• Current national certification as a Nurse Practitioner, with a focus on internal medicine or related fields.
• Active and unrestricted state nursing license in New York.
• A Master’s degree in nursing from an accredited institution.
• A minimum of three years of clinical experience, preferably within community health or public health contexts.
• Exceptional interpersonal and communication skills, demonstrating the ability to build rapport with a diverse patient population.
• A steadfast commitment to promoting social justice and health equity, particularly for vulnerable communities.

Preferred Experience

Compensation and Benefits

This position offers a competitive salary range of $120,000 to $130,000 per year, commensurate with qualifications and experience. In addition, we provide a comprehensive benefits package that includes medical, dental, and vision insurance, a retirement savings plan, and generous paid time off.
