Work Environment

This role will be conducted onsite at our assisted living facility, allowing for direct interactions with residents and their families. Working closely with nurses, social workers, and occupational therapists, the Nurse Practitioner plays a vital role in ensuring seamless communication and care delivery.

Key Responsibilities

• Prescribe medications and treatments in accordance with established medical guidelines and New York state regulations, ensuring safe and effective care for all residents.
• Conduct minor surgical procedures and assist during major surgical interventions, collaborating closely with physicians and other healthcare providers.
• Perform comprehensive assessments of residents’ health conditions, making accurate diagnoses and developing individualized treatment plans that address both physical and emotional well-being.
• Educate patients and their families about disease prevention, health management, and lifestyle modifications to empower them to make informed health decisions.
• Maintain thorough and accurate patient records, ensuring compliance with privacy and healthcare regulations as well as contributing to quality improvement initiatives.

Patient Population / Client Focus

The Nurse Practitioner will primarily serve residents of the assisted living facility, typically seniors who may have chronic health conditions, mobility limitations, or require assistance with daily living activities. This role is critical in managing the unique health needs of this population, including preventive care, disease management, and end-of-life care, to promote overall well-being and comfort.

Qualifications

• Graduation from an accredited Nurse Practitioner program with a Master of Science in Nursing (MSN) or a Doctor of Nursing Practice (DNP).
• Board certification in Family Medicine or Internal Medicine, demonstrating a commitment to high-quality healthcare practices.
• An active, unrestricted nurse practitioner license in New York state, ensuring compliance with local healthcare laws and guidelines.
• Authorization to work in the United States, with a background free of any legal or professional limitations.
• Proficient critical-thinking and analytical skills, enabling effective problem-solving and decision-making in diverse healthcare scenarios.

Preferred Experience

Compensation and Benefits

We offer a competitive salary range of $150,000 to $160,000 yearly, based on experience and caseload.
