Nurture Health Nurse Practitioners Services is in search of a highly skilled and compassionate Cardiology Nurse Practitioner to join our dedicated caregiving team in Carmel, New York. This full-time position focuses on providing exceptional medical care to patients with a range of cardiovascular conditions, in conjunction with an interdisciplinary team that includes seasoned cardiologists.

Work Environment

This position is based onsite in an Assisted Living Facility, which provides a unique opportunity to engage closely with our patient population. Key Responsibilities

• Conduct thorough cardiovascular assessments, focusing on history-taking and physical evaluations to identify patient needs and medical concerns.
• Collaborate daily with cardiologists, discussing diagnosis and treatment strategies to ensure optimal patient care.
• Develop individualistic treatment plans for patients based on the assessment of their heart conditions, including medication management and lifestyle interventions.
• Monitor, document, and communicate patient progress effectively, making timely adjustments to treatment plans as necessary, to enhance outcomes.
• Educate patients and their families on the management of heart diseases, emphasizing medication adherence, dietary recommendations, and lifestyle modifications.
• Participate in holistic follow-up care, including evaluating patients’ responses to treatment during scheduled outpatient visits and planning discharges for those transitioning out of hospital care.
• Engage in continuous learning by staying updated on the latest cardiology advancements and integrating evidence-based practices into everyday care.

Patient Population / Client Focus

This role serves a diverse array of patients at the assisted living facility, primarily focusing on older adults who often present with chronic heart conditions such as heart failure, hypertension, and arrhythmias. The practitioner will have the rewarding opportunity to make meaningful contributions to the long-term health and quality of life for these patients, through personalized medical care and health education.

Qualifications

• A Master's degree in Nursing from an accredited institution.
• Current New York State licensure to practice as a Nurse Practitioner (NP).
• A minimum of two years of experience in a cardiology or related healthcare setting, demonstrating a solid background in cardiovascular care.
• Ability to develop strong rapport with patients, combining clinical expertise with empathy and compassion.

Preferred Experience

• Experience working in an assisted living or long-term care facility is highly desirable.
• Proficiency in electronic health record (EHR) systems and familiarity with telehealth technologies.
• Certification in cardiology nursing or related specialty will be an advantage.

Compensation and Benefits

The base salary for this position ranges from $150,000 to $160,000 annually, commensurate with experience and qualifications.
