This position is based in Huntington, New York, where you will work onsite visiting patients in their homes. Key Responsibilities

• Deliver direct patient care, conducting thorough health assessments and managing chronic heart disease patients both in-home and through telehealth options.
• Perform comprehensive and focused physical examinations to evaluate patients’ ongoing conditions, which includes reviewing past medical histories, gathering relevant health information, and observing vital signs.
• Utilize clinical protocols to order and interpret diagnostic tests such as laboratory tests and imaging studies, thereby developing tailored patient care strategies based on findings.
• Prescribe medications and treatments per established guidelines while monitoring effects and making necessary adjustments to maximize therapeutic outcomes.
• Provide education and counseling to patients and their families about disease prevention and health promotion, ensuring that strategies are culturally sensitive and understandable.
• Collaborate with cardiology specialists and other healthcare team members to implement comprehensive treatment plans that consider all aspects of a patient’s health, facilitating seamless transitions in care.
• Participate in professional development activities, such as continuing education and attending pertinent cardiac workshops and conferences.

Patient Population / Client Focus

The Nurse Practitioner in this role will primarily serve patients diagnosed with cardiac conditions, including congestive heart failure, hypertension, and coronary artery disease. You will cater to a diverse population, considering their age, background, and psychosocial factors. This emphasis on personalizing care ensures that each patient's understanding and adherence to treatment plans are maximized, contributing significantly to their overall health management.

Qualifications

• A Master’s Degree in Nursing is required.
• Active Nurse Practitioner license in New York.
• Certification in a relevant specialty area is preferred.

Compensation and Benefits
