Work Environment

This is a full-time, onsite position where you will operate within a concierge medicine model, conducting house calls and providing in-home care. Key Responsibilities

• Conduct thorough health assessments and screenings for patients in various settings, particularly individuals with complex cardiac needs.
• Diagnose and manage both acute and chronic medical conditions, with a focus on cardiac health, including preventive care and wellness promotion.
• Collaborate with multidisciplinary teams to coordinate referrals and follow-up care, ensuring continuity of services for optimal patient outcomes.
• Engage in community outreach by participating in health fairs and public health initiatives, providing education and health screenings to promote cardiac health awareness.
• Act as an advocate for patients' rights, ensuring access to necessary healthcare services while delivering culturally competent care.

Patient Population / Client Focus

This role primarily focuses on providing care to patients experiencing cardiac conditions, including those in skilled nursing facilities and individuals requiring post-acute management. Your objective will be to promote heart health and improve quality of life through comprehensive care, patient education, and timely access to follow-up services.

Qualifications

• A Master’s degree in Nursing from an accredited nursing program.
• An active state nursing license as a Nurse Practitioner.
• Current certification as a Nurse Practitioner, with a preference for those with special training or certification in cardiology.
• A minimum of 3 years of clinical experience, ideally within community health or public health environments.
• Strong interpersonal skills, with an ability to communicate effectively across diverse populations and clinical settings.

Preferred Experience

Compensation and Benefits
