Key Responsibilities

• Assess, diagnose, and provide treatment for a wide range of cardiovascular conditions, ensuring holistic care for each patient.
• Develop and implement individualized treatment plans in collaboration with the healthcare team, reflecting the diverse needs of patients.
• Educate patients and their families on cardiovascular health, treatment options, and lifestyle modifications necessary to enhance preventive care.
• Monitor patient progress closely and adjust treatment plans based on clinical findings and patient feedback to optimize outcomes.
• Consult with patients daily, utilizing electronic medical records to track progress and coordinate care among different specialists.

Patient Population / Client Focus

This role specifically caters to patients residing in an assisted living facility, primarily aged seniors who often experience multiple comorbidities associated with cardiovascular health. You will be instrumental in managing chronic conditions, conducting health screenings, and facilitating access to various cardiology treatments. Providing compassionate care and building trusting relationships with both patients and their families is essential to the role.

Qualifications

• Master of Science in Nursing (MSN) with a concentration on cardiology or advanced Family Nurse Practitioner program that includes cardiology coursework.
• Cerfified Nurse Practitioner status, preferably with a Cardiology Certified Nurse Practitioner (CCNP) credential from the Cardiology Nursing Certification Board (CNBC).
• A minimum of two years of professional experience as a Nurse Practitioner, specifically in cardiology, primary care, or a related healthcare field.
• Exceptional communication skills and the ability to work collaboratively in a fast-paced healthcare environment.
• Effective clinical skills, with a focus on patient-centered care and evidence-based practice.

Preferred Experience
