Key Responsibilities

• Conduct thorough evaluations and assessments of cardiology patients, including initial physical examinations and history taking.
• Develop and implement evidence-based treatment plans tailored to individual patient needs, coordinating care as necessary.
• Perform and interpret diagnostic tests such as EKGs and echocardiograms, ensuring timely communication of results to patients.
• Maintain comprehensive medical records, documenting patient histories, examinations, and treatment plans accurately in our electronic health record (EHR) system.
• Engage with patients and their families to provide education on heart health, lifestyle modifications, and medication management.
• Make appropriate referrals to specialists as needed, ensuring continuity of care.
• Participate in regular team meetings and case discussions to enhance collaborative care practices.

Patient Population / Client Focus

The Nurse Practitioner will primarily serve an adult population with various cardiology-related conditions, including hypertension, heart failure, and arrhythmias. Qualifications
