Key Responsibilities

• Facilitate all aspects of patient care including hospital admissions, transfers, and discharges, ensuring seamless transitions and continuity of care.
• Coordinate outpatient resources and home-care services, maintaining clear communication with patients and their families.
• Direct patient care activities such as teaching, counseling, and discharge planning, while effectively delegating tasks to other healthcare team members.
• Conduct phone triage for acute clinical issues, providing assessments, and prescribing treatments as necessary based on established protocols.
• Assist in the development, implementation, and evaluation of care standards in collaboration with interdisciplinary care teams, ensuring alignment with institutional policies and regulatory guidelines.
• Engage in ongoing training and education, contributing to the advancement of best practices within the facility.

Qualifications

Compensation and Benefits
