Work Environment

As a Nurse Practitioner at Happy Haven Health Center, you will work onsite in our modern facility that fosters a culture of teamwork and compassion. The typical workday runs from 9:00 a.m. to 6:00 p.m., with some flexibility for evening clinics until 8:00 p.m. Occasional weekend participation in community outreach events and health fairs is expected, contributing to an engaging and dynamic work environment. An average workweek totals approximately 40 hours.

Key Responsibilities

• Conduct thorough health assessments and screenings for community members, identifying both acute and chronic medical conditions.
• Develop and implement comprehensive treatment plans that promote health and prevent illness.
• Collaborate with other healthcare professionals and community organizations to deliver effective health education initiatives.
• Provide culturally competent care, advocating for patients' rights and ensuring they have access to necessary healthcare services.
• Coordinate referrals and follow-up care, ensuring patients receive continuity of care throughout their treatment journey.
• Document patient information meticulously, maintaining accurate medical records to comply with healthcare regulations.
• Engage in community health outreach by organizing and participating in health fairs, education sessions, and local health initiatives.
• Stay informed about the latest community health trends, evidence-based practices, and healthcare policies to enhance service delivery.

Qualifications

• Possession of a current Nurse Practitioner certification.
• Active nursing license for the state of New York.

Compensation and Benefits
