Work Environment

This position is based in a vibrant hospital setting in New York, where healthcare professionals work collaboratively as part of a multidisciplinary team. The work schedule is designed to promote a positive work-life balance, generally from 9:00 a.m. to 6:00 p.m., with occasional evening clinic hours extending until 8:00 p.m., and potential weekend engagement in community outreach events. Key Responsibilities

• Conduct thorough health assessments and screenings for patients of all ages, utilizing clinical expertise to identify their health needs.
• Diagnose and manage both acute and chronic medical conditions, ensuring comprehensive treatment plans that encompass preventive care and health promotion strategies.
• Collaborate with community-based organizations to design and execute health education programs aimed at improving health literacy among residents.
• Deliver culturally sensitive patient care while advocating for patients' rights, particularly for marginalized populations facing barriers to healthcare access.
• Coordinate referrals and meticulously document patient follow-up care, ensuring a seamless continuum of services for optimal health outcomes.

Qualifications

• Current certification as a Nurse Practitioner, with a strong commitment to ethical and patient-focused care.
• An active nursing license in New York State.

Compensation and Benefits
