Work Environment

This role is based onsite at an assisted living facility in Lancaster, NY, where you will work in a collaborative and supportive environment with a multi-disciplinary team. Your work schedule will typically include hours from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. Participation in community outreach activities and health fairs during weekends may also be required. This averages around 40 hours per week, offering a balanced work-life integration.

Key Responsibilities

• Conduct thorough health assessments and physical examinations for individuals and families, developing personalized care plans based on their unique health needs.
• Diagnose and manage both acute and chronic medical conditions; for example, you may oversee treatment plans for diabetes management, hypertension, or respiratory issues, ensuring patients receive timely interventions.
• Engage with local community organizations to design and implement health education programs that address prevalent health concerns, such as nutrition and preventive care.
