Key Responsibilities

• Patient Assessment: Conduct thorough assessments and diagnosis of both acute and chronic medical conditions, utilizing advanced clinical skills to inform treatment decisions.
• Treatment Planning: Develop, implement, and manage individualized treatment plans. Order necessary diagnostic tests and interpret their results to facilitate comprehensive patient care.
• Prescribing: Prescribe medications and therapies tailored to patient needs while monitoring their effects and adjusting as necessary.
• Patient Education: Empower patients and their families through education on health promotion, disease prevention, and management strategies.
• Interdisciplinary Collaboration: Work closely with a cohesive team of healthcare professionals, ensuring a coordinated approach to patient management. Participate in case discussions and multidisciplinary meetings.
• Clinical Documentation: Maintain accurate, up-to-date medical records and documentation in accordance with established guidelines and best practices.
• Quality Improvement: Engage in quality improvement initiatives within the facility, contributing to ongoing enhancements in patient care delivery.
• Regulatory Compliance: Stay informed about the latest healthcare practices and regulatory changes that impact patient care and organizational procedures.

Patient Population / Client Focus

The successful Nurse Practitioner will work primarily with diverse patient populations visiting our outpatient family medicine office. This includes individuals of all ages, each presenting unique healthcare needs and family dynamics. Our focus revolves around continuity of care and fostering strong patient-provider relationships. You will be instrumental in helping patients understand their health conditions and empowering them to participate actively in their health management.

Qualifications

• Current active certification as a Nurse Practitioner.
• Possession of an active state nursing license in New York.
• Bachelor's Degree in Nursing; a Master’s Degree in Nursing is required.
• Minimum of 3-5 years of clinical experience in a relevant specialty, preferably in family medicine or outpatient care.
• Exceptional communication and interpersonal skills to effectively interact with patients, families, and colleagues.
• Strong analytical skills with the ability to solve complex problems in a fast-paced environment.

Preferred Experience

Experience working in a hospital setting is highly desirable, as is proficiency in electronic health record systems. Familiarity with patient-centered medical home models and integrated care is a plus. Additionally, involvement in quality improvement or patient safety initiatives will be considered favorably.

Compensation and Benefits

The salary range for this Nurse Practitioner position is competitive, ranging from USD 150,000 to 160,000 per year , commensurate with experience and qualifications.
