Work Environment

As part of a collaborative care team, the Nurse Practitioner will work alongside physicians, physician assistants, medical assistants, and a dedicated medical scribe. Key Responsibilities

• Provide exceptional primary care services, tailoring treatment plans to the unique needs of each patient.
• Conduct thorough office visits addressing both routine health concerns and acute issues to deliver immediate care.
• Administer annual wellness visits and comprehensive health risk assessments, utilizing a holistic approach to monitor and improve patient health.
• Coordinate care with other healthcare providers, specialists, testing facilities, and agencies to streamline patient access to services.
• Lead population health initiatives in collaboration with the care team to ensure compliance with recommended screenings and preventive care.
• Assist the care team with patient phone triage and outreach, ensuring timely follow-up and education regarding health conditions.
• Educate patients on their health conditions, treatment plans, and self-management strategies to promote better health awareness.
• Engage in promotional activities, representing the clinic at community events and health fairs.
• Conduct home visits when necessary, ensuring continuity of care for patients with limited mobility.
• Report directly to the Medical Director and work closely with operational leadership to enhance care protocols.

Patient Population / Client Focus

This role primarily focuses on older adults who are enrolled in Medicare, emphasizing personalized care plans that enable them to maintain their independence and live healthier lives. Each patient interaction is guided by the principle of quality care, ensuring that their health needs are prioritized and met consistently.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through recognized organizations such as AANP or ANCC.
• Possession of an active, non-probationary Nurse Practitioner license in the state of New York.
• Current DEA license.
• Prior experience in primary care settings, including internal medicine, geriatrics, or family medicine.
• Familiarity with HCC (Hierarchical Condition Categories) documentation, ICD-10 coding, and Health Risk Assessments (HRAs) to ensure accurate patient care records.
• A strong passion for teamwork and a collaborative spirit, essential in this multidisciplinary healthcare environment.

Preferred Experience

• A proven track record of working effectively within a care team in an outpatient setting.
• Advanced proficiency in electronic health record (EHR) systems to manage patient documentation efficiently.

Compensation and Benefits

The annual salary for this Nurse Practitioner role ranges from USD 120,000 to 160,000, dependent on experience and qualifications.
