Position Overview

Atlantis Athletes Physical Therapy Care is seeking a dedicated and compassionate Full-Time Nurse Practitioner to join our team in New York City. In this role, you will provide exceptional primary care to a diverse patient population, focusing primarily on older adults enrolled in Medicare. Work Environment

This full-time position is located onsite in a school setting in New York, New York. You will be part of an integrated care team that includes physicians, nurse practitioners, physician assistants, medical assistants, dedicated scribes, and registered nurses. Our facilities are designed to foster collaboration and transparency, enabling our professionals to provide efficient and coordinated care.

Key Responsibilities

• Deliver high-quality primary care services, conducting thorough office visits for both routine assessments and acute health issues.
• Administer comprehensive annual wellness visits and health risk assessments, documenting findings with precision to support ongoing patient care.
• Coordinate care with external providers, specialists, testing facilities, and relevant agencies to ensure continuity of care.
• Lead population health initiatives alongside the care team, ensuring that eligible patients receive appropriate screenings and preventive interventions.
• Assist the care team with phone triage and outreach to keep patients engaged and informed about their health.
• Educate patients regarding their health conditions, treatment options, and care plans, fostering an environment of trust and understanding.
• Participate in community outreach and promotional events to enhance our visibility and commitment to patient health.
• Conduct home visits when necessary to provide convenient and patient-centered care.
• Collaborate closely with the Medical Director and operational leadership to advance organizational objectives.

Patient Population / Client Focus

Our primary focus is on older adults covered by Medicare who require ongoing medical care and preventive services. We strive to establish personal relationships with each patient, allowing for comprehensive, longitudinal care that helps our patients feel understood and empowered in their health journeys.

Qualifications

• A Master of Science in Nursing, a Doctor of Nursing Practice, or completion of an accredited NP Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through either the AANP or ANCC.
• Possession of an active, non-probationary state Nurse Practitioner license and an active DEA license.
• Demonstrated experience in primary care settings, including internal medicine, geriatrics, or family medicine.
• Familiarity with HCC documentation, ICD-10 coding, and conducting Health Risk Assessments (HRAs).
• A strong enthusiasm for teamwork and a collaborative approach to patient care.

Preferred Experience

• Experience working within a school-based health setting or familiarity with the unique needs of school-aged individuals and their families.
• Knowledge of community health resources and how to leverage them for improved patient outcomes.
• Previous experience in using electronic health records (EHR) to manage patient information efficiently.

Compensation and Benefits

We offer a competitive salary ranging from USD 120,000 to 160,000 annually, commensurate with experience and qualifications. In addition to salary, we provide a comprehensive benefits package that includes health insurance, paid time off, and opportunities for continuing education.
