Work Environment

You will work onsite at our Cicero facility, primarily providing in-home healthcare services. The work schedule operates between 9:00 a.m. and 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Some weekend availability is required for community outreach events and health fairs, fostering a strong connection between our services and the surrounding community. Expect an average workweek of approximately 40 hours as you engage deeply with patients and local groups.

Key Responsibilities

• Conduct thorough health assessments and screenings for diverse individuals and families, ensuring a comprehensive understanding of their health status.
• Diagnose and manage both acute and chronic medical conditions, with a strong emphasis on preventive care and proactive health promotion strategies.
• Collaborate with local community organizations and stakeholders to design, implement, and assess health education initiatives aimed at improving health literacy.
• Provide culturally sensitive care, ensuring that patient rights are respected and advocating for their access to necessary healthcare services.
• Coordinate all aspects of referrals and follow-up care to maintain continuity, including managing appointments and connections to specialists when required.
• Maintain accurate and detailed medical records that comply with relevant regulatory standards, ensuring that all patient encounters are documented appropriately.
• Engage in community outreach events and health fairs, offering free screenings and health consultations to raise awareness about available health services.
• Stay informed about community health trends, infectious disease outbreaks, and evidence-based practices to enhance service delivery.

Qualifications

Compensation and Benefits

Our competitive salary for this role ranges from $180,000 to $200,000 annually, commensurate with experience and qualifications.
