Full-Time Nurse Practitioner (NP) - School-Based Health Services

Work Environment

This position is based onsite in a school facility in Lancaster, New York. The work environment fosters collaborative interaction with school staff, families, and the community. Key Responsibilities

As a Nurse Practitioner, you will be responsible for a range of clinical duties, including:

• Maintaining timely and accurate documentation within our electronic medical record (EMR) software to ensure compliance and continuity of care.
• Collaborating with local and regional clinical leadership to align with strategic healthcare goals, particularly focused on value-based care initiatives.
• Delivering comprehensive patient assessments, diagnosing, and managing both chronic and acute health conditions, with a strong emphasis on health education.
• Coordinating care among various healthcare providers, including telemedicine, specialists, and primary care physicians, to ensure holistic care delivery.
• Leading and participating in family conferences to provide advanced care planning and education tailored to individual member needs.
• Conducting regulatory-required visits, including monthly and quarterly follow-ups, to ensure all patients receive necessary monitoring and care.
• Addressing pharmaceutical recommendations and collaborating with pharmacists to optimize residents' medications.
• Attending to facility needs by providing direct face-to-face patient coverage during work hours.

Patient Population / Client Focus

You will primarily serve students and their families within the school setting. The population may include individuals from diverse backgrounds with unique health challenges. A compassionate approach in addressing the health needs of various demographic groups, particularly low-income and at-risk populations, is key.

Qualifications

• A Bachelor’s Degree in Nursing is required; advanced degrees such as a Master of Science in Nursing or Doctor of Nursing Practice preferred.
• Active DEA license and unrestricted state license to practice as a Nurse Practitioner.
• National certification from AANP or ANCC.
• Valid US work authorization.
• Demonstrated ability to work flexibly and adaptively in a dynamic healthcare environment.
• A genuine passion for providing healthcare to senior and low-income populations.

Preferred Experience

While we welcome applications from NPs of all experience levels, those with prior experience in school health, geriatric care, or community health settings will find this role particularly rewarding. Familiarity with telemedicine practices and EMR systems is advantageous.

Compensation and Benefits
