Key Responsibilities

• Conduct thorough medical histories and physical exams, utilizing evidence-based practices to document pertinent findings and initiate comprehensive care plans.
• Order and interpret diagnostic tests, including laboratory work and imaging studies, and utilize clinical judgment to establish treatment plans and differential diagnoses.
• Facilitate patient admissions and discharges, ensuring smooth transitions between inpatient and outpatient care, and coordinate necessary home healthcare services when required.
• Manage care coordination by prioritizing patient education and self-management strategies, including counseling on disease processes and treatment adherence.
• Utilize triage protocols to assess acute clinical issues over the phone and provide necessary treatment or referrals based on established guidelines.
• Participate in the development and implementation of clinical standards and best practices, collaborating with other healthcare providers to promote optimal patient outcomes.
• Fulfill additional duties as assigned, contributing to the overall success of the healthcare team and the organization’s mission.

Patient Population / Client Focus

As a Nurse Practitioner, you will engage with a diverse patient population exhibiting a wide range of medical needs. Qualifications

• Current New York State Registered Nurse (RN) License.
• National Nurse Practitioner Board Certification, either as a Family Nurse Practitioner (FNP) or Pediatric Nurse Practitioner (PNP).
• DEA License to prescribe medications.
• Basic Life Support (BCLS) and Advanced Cardiac Life Support (ACLS) certification.
• Proficiency in electronic health records (EHR) and the capability to quickly learn new digital applications.
• Strong organizational skills, attention to detail, and the ability to thrive in a fast-paced environment.
• Valid New York State Nurse Practitioner (NP) License, with the ability to maintain and renew as required.

Preferred Experience

• Previous experience in a hospital or outpatient setting is strongly preferred.
• Proficiency in performing clinical assessments and developing treatment plans for patients with diverse health needs.
• Experience in interdisciplinary collaboration within a healthcare team to enhance patient care and health outcomes.
