Work Environment

The Nurse Practitioner role is based onsite in our outpatient facility in Brookhaven, New York. Key Responsibilities

• Conduct thorough health histories and perform problem-specific examinations reflective of patients’ chief complaints during their visits.
• Order, interpret, and communicate the results of diagnostic tests to inform treatment planning.
• Identify, diagnose, and manage both acute and chronic health issues, adhering to established clinical guidelines.
• Develop and implement comprehensive care plans that include medication management, nutritional advice, exercise recommendations, and traditional nursing interventions.
• Collaborate effectively with various healthcare professionals to optimize patient outcomes and facilitate referrals when necessary.
• Regularly evaluate and document patients' responses to their treatment plans, ensuring continuity of care.
• Conduct follow-up consultations via telephone, manage laboratory results, and triage walk-in patients to ensure timely referrals.
• Advise and act as a clinical resource for colleagues, enhancing overall team effectiveness and patient care quality.
• Support the onboarding process for new nurse practitioners through mentorship and guidance.
• Contribute to the development and continual improvement of agency-specific nurse practitioner practice guidelines.
• Engage in peer reviews to maintain quality assurance and improve the standard of patient care.
• Maintain confidentiality and sensitivity regarding patient information at all times.
• Deliver direct patient care during clinical sessions, enhancing the patient experience.

Patient Population / Client Focus

You will provide care to a diverse patient population, focusing on their unique health needs. Encounters will include preventive care, treatment of illnesses, and management of chronic health conditions. You will play an integral role in coordinating care for patients, ensuring their individual goals are met through tailored health solutions.

Qualifications

• New York State Nurse Practitioner license with certification in Family Health or Adult Health.
• Valid New York State Registered Nurse license is preferred.
• Current National Provider Identifier (NPI) number and Drug Enforcement Administration (DEA) certification.
• A Master's Degree in Nursing.
• Strong interpersonal skills and ability to communicate effectively with a wide range of healthcare professionals and patients.

Preferred Experience

• 3 years of relevant experience in an ambulatory care setting.
• Proven ability to manage a full caseload while maintaining high-quality patient care standards.
• Experience in mentorship or training programs within a healthcare setting.

Compensation and Benefits
