Work Environment

Your work will primarily take place onsite at our assisted living facility in Rockville Center. You will enjoy a supportive, team-oriented environment where collaboration and communication are encouraged. The typical work schedule will be from 9:00 a.m. to 6:00 p.m., with the possibility of some evening clinics until 8:00 p.m. Participation in community outreach events or health fairs during weekends may also be required. The average workload is approximately 40 hours a week.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, utilizing evidence-based practices to tailor care.
• Diagnose and manage acute and chronic medical conditions while emphasizing preventive care and overall health promotion.
• Develop and implement health education programs in collaboration with community organizations to enhance public awareness and self-management.
• Advocate for patients’ rights and secure access to necessary healthcare resources, ensuring culturally sensitive care.
• Coordinate referrals and manage follow-up care to maintain continuity and quality of patient care.
• Document patient encounters and care plans meticulously in compliance with regulatory standards to ensure accurate medical history and treatment plans.

Qualifications

• Active state nursing license and current certification as a Nurse Practitioner.
• A Master’s degree in Nursing from an accredited institution.

Preferred Experience

Compensation and Benefits
