Work Environment

This role offers an onsite work environment within our vibrant health center in New York City. You will enjoy a structured schedule from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending to 8:00 p.m. Participation in community events, such as health fairs during weekends, is also part of this role. With an average workweek totaling approximately 40 hours, you will have a balanced work-life schedule while making a significant impact in the community.

Key Responsibilities

• Conduct thorough health assessments and screenings for diverse patient populations, taking into account individual health histories and cultural backgrounds.
• Diagnose and manage a variety of acute and chronic medical conditions, ensuring effective preventive care and health promotion strategies are in place.
• Collaborate with local community organizations and stakeholders to develop and implement health education initiatives aimed at increasing health literacy.
• Provide culturally sensitive care while advocating for patients’ rights, access to healthcare services, and equitable treatment for all.
• Coordinate referrals and follow-up care actively, maintaining continuity of care for patients with complex needs.
• Maintain comprehensive, accurate medical records in compliance with established regulatory standards and best practices.
• Engage in community outreach activities, participating in events to offer health screenings and raise awareness about local health issues.
• Regularly update knowledge on community health trends and evidence-based practices to enhance patient care.

Qualifications

• Current certification as a Nurse Practitioner.

Compensation and Benefits

Happy Haven Health Center offers a competitive salary range of $180,000 to $200,000 per year, commensurate with experience and qualifications.
