Work Environment

Your practice will take place onsite at our outpatient facility, allowing you to develop meaningful relationships with your patients and the community. The work schedule will primarily be 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. Participation in weekend community outreach programs, including health fairs, may be required. Our clinic environment is collaborative and supportive, encouraging teamwork among healthcare professionals to provide comprehensive care.

Key Responsibilities

• Conduct Thorough Assessments: Perform detailed health assessments and screenings for individuals and families, utilizing evidence-based practices to support your clinical decisions.
• Manage Ongoing Health Issues: Diagnose and treat both acute and chronic conditions, effectively managing care plans that include preventive measures and health promotion activities tailored to individual patient needs.
• Community Collaboration: Partner with local organizations to create and implement health education initiatives that address prevalent health issues within the community, aiming to empower individuals through knowledge.

Qualifications

• Current certification as a Nurse Practitioner.
• An active New York state nursing license.
