Work Environment

This position is based onsite, primarily in the homes of our patients within the Cicero area. As an FNP, you will work a standard schedule from 9:00 a.m. to 6:00 p.m., with some evenings extending to 8:00 p.m. for clinic sessions, and occasional weekends for community outreach events and health fairs. This role encompasses an average of 40 hours per week and requires flexibility to adapt to the needs of the community and our patients.

Key Responsibilities

• Conduct thorough health assessments and screenings for patients of all ages in a home care setting.
• Diagnose and effectively manage a range of acute and chronic medical conditions, utilizing evidence-based practice for preventive care.
• Develop and implement health education programs in collaboration with community organizations, tailored to address specific public health challenges in the Cicero area.

Qualifications

• Active Nurse Practitioner certification and a valid New York state nursing license.

Compensation and Benefits

We offer a competitive salary range of $180,000 to $200,000 annually, dependent on your experience and qualifications.
