Work Environment

This position will be situated in an Assisted Living Facility in Carmel, a vibrant and serene community in New York. The work schedule will typically range from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. to accommodate our patients' needs. Participation in weekend community outreach events or health fairs may also be required, allowing you to engage directly with the community. This full-time role typically encompasses an average workweek of approximately 40 hours, ensuring a balanced work-life integration.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, ensuring comprehensive care delivery.
• Diagnose and manage both acute and chronic medical conditions, emphasizing preventive care and health promotion strategies.

Patient Population / Client Focus

You will be serving a diverse patient population within the assisted living facility, primarily focusing on elderly individuals and those with complex health needs. Qualifications

• A Master's or Doctoral degree in Nursing from an accredited nursing program.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) recognized by the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• Current and unrestricted Advanced Practice Registered Nurse (APRN) license in New York State.
• Possession of a valid DEA number and NPI number is required.
• Strong clinical assessment and diagnostic skills.
• Excellent communication and interpersonal abilities, essential for building relationships with patients and colleagues.
• Ability to function autonomously as well as collaboratively in a team setting.
• Proficiency in using Electronic Health Records (EHR) systems for patient documentation and management.

Preferred Experience

• A minimum of three to five years of clinical experience, preferably in primary care or family medicine, with a strong emphasis on community or public health settings.

Compensation and Benefits

This full-time Nurse Practitioner position offers a competitive salary range of USD 120,000 to 130,000 per year, commensurate with experience and qualifications.
