Work Environment

This role is based in Huntington, New York, operating within an onsite, concierge medicine setting that prioritizes personal and attentive care. Our collaborative healthcare team includes physicians, nurse practitioners, physician assistants, medical assistants, and care managers, all working together to streamline processes and enhance patient experiences. You will have the benefit of working with a dedicated medical scribe, allowing you to focus on patient care rather than administrative tasks.

Key Responsibilities

As a Nurse Practitioner at Brightwood Health, you will:

• Provide exceptional primary care services, assessing and managing routine and acute medical issues.
• Administer annual wellness visits and health risk assessments, utilizing a holistic approach to patient health, requiring thoughtful documentation and follow-up.
• Coordinate care with other healthcare providers, specialists, testing facilities, and community resources to ensure comprehensive patient management.
• Lead population health initiatives, including ensuring timely cancer screenings and managing chronic conditions.
• Assist the care team with phone triage, outreach, and patient education regarding health conditions and treatment plans.
• Conduct home visits as necessary to ensure continuity of care and engagement with patients.
• Participate in promotional and community outreach activities to promote our services.
• Report to the Medical Director and collaborate closely with operational leadership and the care team to enhance service delivery.

Patient Population / Client Focus

Your primary focus will be on older adults enrolled in Medicare, a demographic that often requires more nuanced care due to complexities in health conditions. By establishing trusting relationships and understanding individual patient needs, you will empower patients to engage actively with their healthcare plans, promoting healthier lifestyles and improved quality of life.

Qualifications

The ideal candidate will possess the following qualifications:

• A Master of Science in Nursing or a Doctor of Nursing Practice from an accredited institution.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner from AANP or ANCC.
• An active, non-probationary Nurse Practitioner license in the state of New York.
• A valid DEA license for prescribing medications.
• A proven background in primary care settings, specifically in internal medicine, geriatrics, or family medicine.
• Familiarity with HCC (Hierarchical Condition Categories) documentation, ICD-10 coding, and Health Risk Assessments.

Preferred Experience

While not mandatory, candidates with the following experiences will be highly regarded:
