Work Environment

The position is onsite, providing direct care within the Carmel, NY area. The work setting is a blend of office administration and in-home patient visits, which allows for a flexible, patient-centered approach to care. You will have a structured work schedule primarily from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. Additionally, participation in weekend community outreach events or health fairs may be required, ensuring our services are continuously accessible to those in need.

Key Responsibilities

• Conduct thorough health assessments, including physical exams, patient histories, and health screenings tailored to families and individuals.
• Diagnose and manage both acute and chronic medical conditions, focusing on preventive care and health promotion tailored to diverse patient needs.
• Develop and implement health education programs in collaboration with local community organizations, enhancing public understanding of health issues.
• Provide culturally sensitive care, advocating for patients' rights and healthcare access, ensuring that every patient feels valued and understood.
• Coordinate referrals and follow-up care to ensure continuity and quality of care for all patients.
• Maintain comprehensive, accurate medical records in compliance with healthcare regulations and standards, utilizing advanced electronic health record (EHR) systems.
• Engage actively in community outreach and health fairs to directly provide health screenings and promote wellness within underserved populations.
• Continuously update knowledge of community health trends, evidence-based practices, and healthcare policies relevant to our patient populations.

Patient Population / Client Focus

Your primary focus will be on families and individuals within the Carmel community, particularly those from underserved backgrounds who may face barriers to accessing quality healthcare. By understanding and addressing the unique social determinants affecting these populations, you will help empower patients to take charge of their health and wellbeing.

Qualifications

• Current certification as a Nurse Practitioner, with an active state nursing license in New York.
• A Bachelor's Degree in Nursing is required as a minimum educational qualification; however, a Master's Degree in Nursing from an accredited program is preferred.
• Strong interpersonal and communication skills essential for effectively collaborating with diverse groups and building trust within the community.
• A strong commitment to social justice, health equity, and advocacy for vulnerable communities, ensuring that every patient receives equitable care.

Preferred Experience

• A minimum of 3 years of clinical experience is required, with a preference for candidates who have worked in community health or public health settings.

Compensation and Benefits

• This position offers a competitive salary range of $180,000–$200,000 per year, commensurate with experience and qualifications.
