Work Environment

This is an onsite position at our Carmel, NY clinic. You will work in a collaborative and supportive environment alongside a team of healthcare professionals dedicated to patient advocacy and community service. The clinic operates from 9:00 a.m. to 6:00 p.m., with the flexibility of occasional evening clinics until 8:00 p.m. Weekend participation in community outreach events and health fairs may also be required. An average workweek totals approximately 40 hours, allowing for a balanced work-life integration.

Key Responsibilities

• Conduct thorough health assessments and screenings for patients of all ages to identify health risks and needs.
• Diagnose and manage acute and chronic medical conditions, providing preventive care and health promotion strategies tailored to individuals and families.
• Collaborate with local organizations and community stakeholders to develop impactful health education programs that promote wellness and disease prevention.
• Offer culturally sensitive care while advocating for patients’ rights and access to essential healthcare services.
• Coordinate referrals and follow-up care, ensuring seamless continuity of care for all patients.
• Maintain precise medical records in accordance with regulatory standards, enabling effective communication within our care team.

Patient Population / Client Focus

The Family Nurse Practitioner will serve a diverse patient population, prioritizing care for individuals and families within the Carmel community, particularly those who may experience barriers to accessing traditional healthcare services. Qualifications

• Current certification as a Nurse Practitioner in the state of New York.

Compensation and Benefits

We offer a competitive salary range of $180,000 to $200,000 per year, commensurate with experience and skills.
