Work Environment

Key Responsibilities

• Conduct comprehensive health assessments and screenings for patients, both new and returning, ensuring each individual receives a tailored approach to their health needs.
• Diagnose and manage acute and chronic medical conditions, actively creating preventive care plans and health promotion strategies.
• Collaborate with local community organizations and stakeholders to curate and implement impactful health education programs tailored to the unique challenges faced by our population.
• Provide culturally sensitive care, ensuring that patients’ voices are heard and advocating for their rights within the healthcare system.
• Coordinate patient referrals and follow-up care, meticulously tracking and managing patient progress to ensure continuity and quality of care.
• Maintain detailed and accurate medical records, adhering to all regulatory standards to ensure compliance and facilitate seamless communication between care providers.
• Participate in community outreach events and health fairs, working actively to raise awareness in critical health issues and offering essential health screenings.
• Stay informed on community health trends, evidence-based practices, and policy changes to better serve our patient population.

Patient Population / Client Focus

You will primarily work with a diverse group of patients in an assisted living environment, serving individuals with varying healthcare needs, including those with chronic conditions, disabilities, and age-related health challenges. Your role is pivotal in empowering patients and their families through health education and support, striving to enhance their overall quality of life.

Qualifications

• Current certification as a Nurse Practitioner.
• Active state nursing license in New York.
• A Master’s degree in Nursing from an accredited program.
• Minimum of 3 years of clinical experience, especially in community health or public health settings, is essential.

Compensation and Benefits
