Center Point Healthcare Solution Management Practice is seeking a dedicated and skilled Adult-Gerontology Nurse Practitioner to join our full-time, onsite team in Brookhaven, New York. Work Environment

The Nurse Practitioner will practice primarily within our Assisted Living Facility and outpatient practice settings, where a collaborative care approach is paramount. You will work in a team-oriented environment alongside physicians, other nurse practitioners, physician assistants, registered nurses, and care managers. This multidisciplinary structure allows each team member to focus on their strengths, enhancing the quality of patient care and driving better health outcomes.

Key Responsibilities

• Provide high-quality primary care tailored to the specific health needs of adult and geriatric patients.
• Conduct thorough office visits, addressing both routine and acute health issues.
• Administer annual wellness visits and health risk assessments, ensuring comprehensive documentation that reflects each patient's health status and care plan.
• Coordinate care with other healthcare providers, specialists, and community resources, effectively managing transitions in care.
• Lead initiatives in population health, ensuring preventive screenings such as breast cancer checks are performed according to evidence-based guidelines.
• Support the care team through patient phone triage, outreach, and follow-up communications.
• Provide education to patients regarding their health conditions, treatments, and care plans to foster understanding and adherence.
• Participate in community outreach and promotional activities to elevate awareness of our services.
• Conduct home visits as needed to assess patients' living situations and make appropriate care adjustments.
• Other related duties as assigned and report directly to the Medical Director.

Patient Population / Client Focus

Our primary focus is on Medicare-eligible adults, with a specialized lens toward the geriatric population. Qualifications

• A Master's Degree in Nursing, Doctor of Nursing Practice (DNP), or equivalent Nurse Practitioner program credential.
• National certification as an Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP) from recognized bodies such as AANP or ANCC.
• An active and non-probationary state Nurse Practitioner license.
• An active DEA license for administering controlled substances.
• Demonstrated experience within primary care settings, particularly in internal medicine, geriatrics, or family medicine.
• Knowledge of Hierarchical Condition Categories (HCC) documentation, ICD-10 coding, and health risk assessments.
• A strong affinity for teamwork and the desire to collaborate cross-functionally.

Preferred Experience

• Experience in outpatient primary care settings focusing on adult and geriatric populations.
• A background in health promotion and disease prevention strategies.
• Familiarity with electronic health record (EHR) systems and their application in patient care management.
• Strong communication skills to interact effectively with patients, families, and interdisciplinary teams.
