Work Environment

This position operates onsite, where you will collaborate with a dedicated team of healthcare professionals including physicians, other nurse practitioners, physician assistants, medical assistants, and support staff such as registered nurses and care managers. Key Responsibilities

Your responsibilities as a Nurse Practitioner will encompass a range of clinical duties aimed at promoting patient health and managing conditions in an outpatient setting. Specific duties include:

• Delivering exceptional primary care tailored to adult patients.
• Performing office visits for both routine check-ups and acute health issues, ensuring accurate, thorough assessments and documentation.
• Administering annual wellness visits and conducting comprehensive health risk assessments that require detailed, holistic health evaluations.
• Coordinating care with specialists, testing facilities, and other healthcare agencies to streamline the patient experience.
• Taking the lead in population health initiatives, such as ensuring eligible patients receive necessary screenings (e.g., biennial breast cancer screenings for women).
• Providing assistance with phone triage and outreach to patients for follow-up care.
• Educating patients regarding their health conditions, care plans, and treatment protocols, fostering understanding and adherence to prescribed regimens.
• Participating actively in promotional activities aimed at community education and engagement.
• Conducting home visits as necessary to provide care in a comfortable environment for patients.
• Reporting to the Medical Director while maintaining close cooperation with operational leadership for optimal patient care delivery.

Patient Population / Client Focus

This role primarily focuses on providing comprehensive care to older adults enrolled in Medicare. Understanding their unique health challenges—such as multiple comorbidities and the need for chronic disease management—is crucial in delivering effective and empathetic care. Your expertise in adult-gerontology will be pivotal in ensuring these patients maintain their health and quality of life.

Qualifications

To be successful in this role, candidates must possess the following qualifications:

• A Master of Science in Nursing, Doctor of Nursing Practice, or a completion of an NP Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through the AANP or ANCC.
• An active, non-probationary state Nurse Practitioner license.
• An active DEA license for prescribing medications.
• Prior experience in primary care settings, particularly within internal medicine, geriatrics, or family medicine.
• A robust understanding of HCC (Hierarchical Condition Categories) documentation, ICD-10 coding, and health risk assessments.
• A collaborative spirit, with a passion for teamwork and cross-functional collaboration.

Preferred Experience

While our minimum qualifications specify essential credentials, preference will be given to candidates with extensive experience in chronic disease management and a strong background in team-based care settings. Compensation and Benefits
