Key Responsibilities

• Perform thorough health assessments and screenings, ensuring an accurate understanding of each patient’s medical history and current health status.
• Diagnose and manage both acute and chronic medical conditions, prescribing appropriate treatments and preventive measures tailored to individual patient needs.
• Develop and implement health education programs in collaboration with community organizations, addressing critical health topics and promoting wellness initiatives.
• Advocate for patients' rights and access to healthcare services, ensuring culturally sensitive care that respects the diverse backgrounds of our patient population.
• Coordinate referrals to specialists and ensure effective follow-up care to maintain continuity and quality of healthcare services.
• Maintain up-to-date and accurate medical records that comply with state and federal regulations, enhancing care coordination and communication.

Patient Population / Client Focus

Your focus will be on serving diverse populations in New York, with a particular emphasis on underserved communities who may experience barriers to healthcare access. Qualifications

• Current certification as a Nurse Practitioner, demonstrating your expertise in advanced nursing practices and patient management.
• An active state nursing license valid in New York.
• A Master’s degree in nursing from an accredited institution, ensuring a strong foundation in advanced clinical knowledge.
• At least three years of clinical experience, ideally in community health or public health settings, to effectively address the needs of our patient population.

Compensation and Benefits

The salary for this role ranges from USD 120,000 to 130,000 per year, commensurate with experience and qualifications. Additionally, we offer a comprehensive benefits package that includes:
