Work Environment

This position is rooted in a home care setting, allowing for a unique opportunity to practice medicine in the community context rather than a traditional clinic environment. Working from 9:00 a.m. to 6:00 p.m., with potential evening clinics until 8:00 p.m. and occasional weekend outreach, you will have a dynamic work schedule that keeps you actively involved in community health initiatives. An average workweek totals approximately 40 hours, providing a balance between fulfilling patient care responsibilities and participating in outreach programs.

Key Responsibilities

• Conduct thorough health assessments and screenings for various patients, utilizing evidence-based methodologies tailored to individual needs.
• Diagnose and manage both acute and chronic medical conditions, emphasizing preventive care and health promotion strategies that align with community health objectives.
• Collaborate with local organizations and stakeholders to design and implement robust health education programs aimed at improving public health literacy.
• Provide culturally sensitive care by understanding the unique backgrounds and needs of diverse patient populations and advocating for their access to healthcare services.
• Coordinate referrals and follow-up care to ensure continuity of health services, optimizing patient outcomes and maintaining comprehensive treatment plans.
• Document patient interactions and care plans in detailed medical records in compliance with all regulatory standards and best practices.
• Engage actively in community outreach events and health fairs, contributing to raising awareness and promoting health screenings.
• Stay informed on community health trends, emerging evidence-based practices, and relevant local and federal healthcare policies to enhance service delivery.

Qualifications

• Current certification as a Nurse Practitioner is essential.
• An active state nursing license is mandatory.

Compensation and Benefits

We offer a competitive annual salary range of $180,000 to $200,000, dependent on experience and qualifications.
