Work Environment

You will work onsite at our outpatient office in Forest Hills, NY, where a passionate team of healthcare professionals collaborates to deliver exceptional care. Our work setting is designed to foster open communication and teamwork, ensuring that every member contributes to improving patient care and community health initiatives. You can expect a structured work schedule with regular hours, primarily from 9:00 a.m. to 6:00 p.m., with some required evening shifts until 8:00 p.m. and occasional weekend events.

Key Responsibilities

• Conduct thorough health assessments and screenings for patients, focusing not just on medical needs but also incorporating mental health evaluations to provide holistic care.
• Diagnose and manage a range of acute and chronic conditions, implementing advanced practice nursing protocols and evidence-based strategies for health promotion.
• Engage with community stakeholders to design and implement health education programs that address pressing health issues and improve community welfare.
• Provide culturally competent care, ensuring that all patients feel respected and understood, while advocating for equitable access to services.
• Coordinate referrals to appropriate specialists and ensure effective follow-up care to maintain continuity of treatment and patient engagement.
• Document patient interactions comprehensively, adhering to all regulatory and organizational standards for medical records accuracy.
• Participate in health fairs and community outreach efforts, highlighting services and providing health screenings that benefit underserved populations.
• Stay abreast of the latest health trends, best practices, and regulatory updates to ensure that the care provided is both current and relevant.

Patient Population / Client Focus

The Nurse Practitioner in this role will work with a diverse population, including individuals facing mental health challenges, chronic illness, and social determinants of health. Qualifications

• Current certification as a Nurse Practitioner is required, along with an active nursing license in New York State.
• A minimum of a Master's Degree in Nursing from an accredited institution is mandatory.
• Demonstrated commitment to community health with at least three years of clinical experience, particularly in public health or community-based settings.
• Exceptional interpersonal and communication skills are essential, with the ability to relate effectively to diverse populations.

Compensation and Benefits

We offer a competitive annual salary ranging from USD 180,000 to 200,000, based on qualifications and experience.
