Work Environment

This role is primarily conducted on-site within our outpatient facility located in the heart of New York, New York. The work setting is engaging and collaborative, with a focus on team-based health care delivery. Your schedule will generally range from 9:00 a.m. to 6:00 p.m., with the potential for occasional evening clinics until 8:00 p.m., enabling flexibility to meet the community’s needs. Participation in community outreach initiatives and health fairs during weekends may also be required, enriching your connection with the community.

Key Responsibilities

• Conduct comprehensive health assessments and screenings to address the diverse healthcare needs of individuals and families.
• Diagnose and manage both acute and chronic medical conditions while delivering preventive care and health promotion.
• Collaborate with local organizations to develop and facilitate health education programs that empower the community with knowledge about wellness and self-care.
• Provide culturally sensitive care and actively advocate for patients' rights and equitable access to healthcare services.
• Coordinate patient referrals and follow-up care to assure the continuity of patient management and support.
• Maintain meticulous and accurate medical records in compliance with all regulatory standards, ensuring high-quality patient care.
• Engage in community outreach events, enhancing awareness and providing essential health screenings and education on public health issues.
• Stay informed on community health trends and evidence-based practices to continuously improve patient care quality.

Patient Population / Client Focus

This role serves a diverse patient population in New York City, characterized by varying socio-economic backgrounds and health needs. As a Nurse Practitioner in this environment, you will be integral in addressing challenges faced by underserved communities, utilizing your skills to foster improved health outcomes and advocate for positive change.

Qualifications

• A Master’s or Doctoral degree in Nursing from an accredited program.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) by the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• Current, unrestricted Nurse Practitioner (APRN) license in New York State.
• DEA# and NPI# are required for this position.
• Strong clinical assessment capabilities along with excellent communication and interpersonal skills.
• Aptitude for working independently while also being an effective team player.
• Proficiency in electronic health records (EHR) systems to maintain accurate patient documentation.

Preferred Experience

• A minimum of 3-5 years of clinical experience, preferably in community health or public health settings.
• Experience in primary care or family medicine is highly desirable, providing a solid foundation for addressing community health issues.
• Proven ability to interact effectively with a diverse population, enhancing communication and relationship-building.

Compensation and Benefits

The annual salary for this role ranges from USD 150,000 to 160,000, reflecting your expertise and the impact of your contributions to community health. Additionally, a sign-on bonus of up to $10,000 is available for qualified candidates.
