Community Health Nurse Practitioner - Full-Time Onsite Role

Position Overview

We are seeking a dedicated and compassionate Community Health Nurse Practitioner to join our team at the US Veterans Health Care Clinic in Lancaster, New York. The ideal candidate will work onsite Monday through Friday from 9 AM to 5 PM, with a salary range of USD 150,000 to 160,000 annually, aligned with experience and qualifications.

Key Responsibilities

• Collaborate with community organizations and stakeholders to design and implement health education programs that address the specific needs of the veteran population.
• Provide culturally sensitive care to individual veterans and their families, ensuring advocacy for patients' rights and equitable access to healthcare services.
• Maintain comprehensive and accurate medical records in compliance with all regulatory standards, utilizing electronic health record (EHR) systems for efficient documentation and data management.
• Conduct health screenings and participate in community outreach initiatives, such as health fairs, to raise awareness about public health issues affecting veterans.
• Stay current with community health trends, best practices, and applicable healthcare policies to inform practice and elevate patient care standards.

Patient Population / Client Focus

This role focuses on providing care to a diverse range of veteran populations, including those with chronic conditions, mental health issues, and varying socioeconomic backgrounds. As a Nurse Practitioner, you will tailor your clinical approach to empower patients by educating them about their health conditions and treatment options, thereby enhancing their ability to manage their own health effectively.

Qualifications

• A Master’s or Doctoral degree in Nursing from an accredited institution.
• Current certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (ANP) by the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• A valid, unrestricted Nurse Practitioner (APRN) license in New York State.
• DEA and NPI numbers are required to facilitate medication management.
• Strong clinical assessment and diagnostic skills are essential to provide high-quality care tailored to complex veteran needs.

Preferred Experience

• A minimum of 3-5 years of clinical experience, ideally in community health or public health settings.
• Demonstrated ability to work effectively with diverse populations, showcasing strong interpersonal and communication skills.
• Experience with EHR systems and community health programming preferred.
