We are inviting applications for a full-time Advanced Cardiac Care Nurse Practitioner at the Jewish Community Health Center located in Huntington, New York. This vital role involves working within the outpatient practice of our Center for Advanced Cardiac Care (CACC), where you will collaborate with a dedicated team focused on providing high-quality cardiology services to our community.

Work Environment

This role is positioned in the fast-paced, dynamic environment of a hospital outpatient setting. You will be working onsite alongside a diverse team of specialists, including cardiologists, nurse practitioners, and administrative support staff, all committed to improving patient health through multidisciplinary collaboration. The work schedule is Monday through Friday from 9 AM to 5 PM, with no on-call duties, ensuring a work-life balance that supports your personal life.

Key Responsibilities

• Collaborate closely with cardiologists to provide comprehensive care to patients, ensuring effective management of cardiovascular diseases.
• Conduct thorough patient assessments, including history-taking and physical examinations, to determine appropriate treatment plans.
• Manage patients suffering from chronic conditions such as hypertension and high cholesterol, providing education on lifestyle modifications and medication adherence.
• Deliver specialized outpatient Electrophysiology services to patients with arrhythmias and those requiring electrophysiological device management.
• Handle acute conditions, including Coronary Artery Disease, Congestive Heart Failure, and Transient Ischemic Attacks, employing evidence-based practices.
• Administer stress tests to evaluate the cardiovascular condition of patients, interpreting results in collaboration with cardiologists.
• Prescribe medications and adjust treatment plans based on symptoms and test results, always ensuring compliance with clinical guidelines.
• Provide patient education for chronic disease management and wellness, engaging family members in discussions regarding care plans and treatment options.
• Refer patients to relevant specialists within the healthcare group for advanced care as necessary.
• Stay updated on best practices in cardiology through ongoing education and professional development opportunities.

Patient Population / Client Focus

You will serve a diverse patient population, including individuals with various cardiovascular conditions, many of whom require ongoing management of chronic diseases. Our focus is on enhancing the quality of life for patients through preventive care, education, and efficient management of their cardiovascular health.

Qualifications

• A Bachelor’s degree in nursing from an accredited institution.
• Must be a graduate of a Nurse Practitioner program and possess the necessary certifications to practice in New York state.
• A current and valid Nurse Practitioner license.
• Possession of a Master’s or Doctoral degree is highly desirable.

Preferred Experience

• Prior experience in cardiology, whether in inpatient or outpatient settings, is preferred.
• Familiarity with cardiac procedures and the ability to interpret cardiology-related diagnostic tests.
• Experience in managing a patient caseload effectively and efficiently.

Compensation and Benefits
