Work Environment

This position is set in a dynamic home care environment, where you will have the chance to work closely with patients in their own homes as well as within our healthcare facility. You will follow a structured Monday through Friday schedule, with working hours from 9:00 AM to 5:00 PM, and the responsibility of working two Saturdays a month from 9:00 AM to 5:00 PM. This flexible yet structured schedule allows for a balanced approach to patient care, enabling you to efficiently manage your caseload while ensuring enhanced quality time with each individual.

Key Responsibilities

• Manage a daily caseload of 16-20 patients, ensuring efficient and high-quality encounters that are tailored to meet each patient's needs.
• Conduct comprehensive initial evaluations and regular follow-up visits, continuously tracking patient progress and making adjustments to care plans as necessary.
• Utilize your expertise in interpreting chest X-rays and analyzing stress test results to inform treatment decisions and enhance diagnostic accuracy.
• Gather detailed patient histories, focusing on clinical symptoms and relevant medical factors to facilitate accurate diagnoses.
• Perform focused physical examinations, identifying critical health indicators that contribute to successful treatment plans.
• Diagnose and manage a wide array of health issues, including hypertension, diabetes, COPD, and CHF.
• Provide specialized care for patients recovering from myocardial infarctions, supporting their rehabilitation and optimal recovery.
• Educate patients on lifestyle modifications, nutrition, and preventive strategies to promote cardiovascular health and wellness.
• Effectively interpret and manage cardiac diagnostic tools, ensuring data is integrated into patient care decisions.
• Coordinate with a multidisciplinary team to create comprehensive, individualized treatment plans that focus on improving patient outcomes.

Patient Population / Client Focus

You will primarily work with adult patients experiencing a variety of cardiovascular issues, including chronic conditions and post-acute care. Building meaningful relationships with your patients will be paramount as you guide them through their healthcare journey, considering the complexities and nuances of their individual health situations.

Qualifications

• Active Nurse Practitioner license in the state of New York with current DEA registration.
• A Master of Science in Nursing (MSN) with Family Nurse Practitioner (FNP), Adult Nurse Practitioner (ANP), or Adult-Gerontology Nurse Practitioner (AGNP) certification.
• A minimum of 2 years of clinical experience as a Nurse Practitioner or Advanced Practice Registered Nurse (APRN).
• Exceptional problem-solving skills and strong organizational abilities are essential to manage the diverse daily responsibilities.

Preferred Experience

• Experience in managing patients with chronic cardiac conditions will be highly valued.
• Previous experience in outpatient care settings is preferred, enhancing your ability to navigate home care environments effectively.

Compensation and Benefits
