Key Responsibilities

• Conduct thorough initial patient evaluations through comprehensive physical examinations and health assessments.
• Develop, implement, and manage personalized treatment plans tailored to the unique needs of each patient.
• Monitor daily health status, manage chronic cardiovascular conditions, and provide preventive care.
• Utilize advanced medical equipment safely, operating and maintaining instruments for various diagnostic tests.
• Order, perform, and interpret diagnostic studies such as ECGs and lab tests, relaying significant results to patients and their families in understandable terms.
• Document detailed patient medical histories and updates accurately in electronic medical records (EMR).
• Coordinate referrals to specialists or ancillary services when necessary, ensuring continuity of care.
• Engage in patient education efforts, emphasizing lifestyle modifications and adherence to treatment plans for heart health.
• Participate in quality improvement initiatives aimed at enhancing patient care and outcomes within the facility.

Patient Population / Client Focus

Your primary focus will be on elderly residents who often present with complex comorbidities, including various cardiovascular diseases. Qualifications

• Current certification as a Nurse Practitioner in the state of New York.
• Active and unrestricted New York state nursing license.
• Master's degree in nursing from an accredited institution.
