Work Environment

The Nurse Practitioner will work onsite in a well-equipped, modern facility that promotes a patient-centered approach to care. Key Responsibilities

• Conduct comprehensive health assessments and screenings for residents, identifying needs and formulating care plans.
• Administer stress tests, order chest X-rays, and interpret diagnostic medical imaging results to support accurate diagnoses.
• Diagnose and manage conditions such as Peripheral Arterial Disease (PAD), hypertension, diabetes, and high cholesterol, developing personalized treatment plans.
• Implement medication management strategies, particularly for patients with Congestive Heart Failure (CHF) and Atherosclerosis, ensuring optimal therapeutic outcomes.
• Collaborate with community organizations and healthcare stakeholders to promote health education programs within the assisted living population.
• Advocate for residents' rights and access to healthcare services, providing culturally sensitive care that respects their individual backgrounds.
• Coordinate referrals and follow-up care, ensuring continuous support through the healthcare journey.
• Maintain detailed and accurate medical records in compliance with regulatory standards and organizational policies.
• Participate actively in community outreach events and health fairs to raise health awareness and provide valuable health screenings to the public.
• Stay abreast of community health trends, best practices, and relevant healthcare policies to continually enhance care offerings.

Patient Population / Client Focus

This role will focus primarily on a diverse population of older adults who reside in the assisted living facility. Many of our residents may have chronic health conditions requiring regular monitoring and management, as well as varying degrees of mobility and cognitive function. The Nurse Practitioner will play a critical role in addressing the unique healthcare needs of this community, enhancing their overall quality of life and well-being.

Qualifications

The ideal candidate will possess the following qualifications:

• A Bachelor’s Degree in Nursing or related field.
• A valid Nurse Practitioner license in the state of New York.
• A minimum of 1 year of relevant clinical experience in a healthcare setting, preferably within cardiology or geriatric care.

Preferred Experience

While we welcome new graduates, candidates with experience in managing chronic conditions and working within assisted living environments are highly preferred. Familiarity with electronic health record (EHR) systems and patient management software will be advantageous.

Compensation and Benefits

This full-time position offers a competitive salary range of USD 120,000 to 140,000 annually, commensurate with experience.
