Key Responsibilities

• Conduct thorough assessments of patients, establishing diagnoses and managing both acute and chronic health conditions within the scope of practice as defined by New York State law.
• Perform comprehensive health histories and physical examinations, meticulously documenting all findings in patient records.
• Order and interpret necessary diagnostic tests, including laboratory and radiological studies, to support decision-making processes.
• Develop individualized treatment plans that incorporate both pharmacologic and non-pharmacologic strategies for optimal health outcomes.
• Collaborate with interdisciplinary teams to establish patient-centered goals, monitor effectiveness of interventions, and adapt treatment plans based on ongoing evaluations.
• Provide education to patients and their families regarding health promotion, chronic disease management, and preventive care strategies.
• Participate in quality improvement initiatives and contribute to clinical protocols that enhance healthcare delivery within our practice.

Patient Population / Client Focus

Our Internal Medicine Nurse Practitioner will primarily focus on adult patients, many of whom present with complex medical histories and chronic conditions such as diabetes, hypertension, and cardiovascular diseases. The practitioner will play a vital role in managing these conditions through comprehensive care strategies and dedicated follow-up, ensuring that all patients receive the support necessary to improve their health outcomes.

Qualifications

• A Master’s Degree in Nursing with a focus on Nurse Practitioner studies is required.
• Valid New York State Registered Professional Nurse and Nurse Practitioner licenses.
• Certification from a recognized Nurse Practitioner board.
• Current BCLS and ACLS certifications to ensure competency in critical care skills.
• Strong clinical, interpersonal, and communication skills are essential to effectively interact with a diverse patient population and multidisciplinary teams.

Preferred Experience

• A minimum of 3 years of advanced practice experience in an internal medicine setting, specifically caring for adult patients, is preferred.

Compensation and Benefits
