Work Environment

This position is based onsite at our CACC, providing a supportive and engaging work environment. As a Nurse Practitioner, you will work closely with a talented team of Physicians, fellow NPs, Registered Nurses, and various specialists to ensure comprehensive care for our patients. Key Responsibilities

Your primary responsibilities will include:

• Obtaining detailed patient histories and conducting thorough physical examinations to assess patient health comprehensively.
• Ordering and interpreting diagnostic tests while diagnosing and managing both chronic and acute healthcare issues, such as heart disease or arrhythmias.
• Developing and implementing goal-directed care plans in tandem with prescribing appropriate interventions and follow-up protocols.
• Educating patients and their families on disease processes, preventative care strategies, and treatment plans to promote health management outside the clinical setting.
• Working closely with the multidisciplinary team to coordinate care across various services, including admissions, discharges, and outpatient follow-ups.
• Maintaining accurate and up-to-date documentation for all patient encounters within the electronic medical record (EMR) systems.
• Engaging in research protocols, reviewing clinical studies, and integrating findings to refine and enhance clinical practice.
• Upholding high standards of patient care and safety, ensuring a therapeutic and welcoming environment for patients and their caregivers.

Patient Population / Client Focus

The NP will focus primarily on adult populations dealing with complex cardiovascular conditions, including patients with advanced heart failure, pulmonary arterial hypertension, and those post-heart transplant. This clinical role requires an empathetic approach, as you will be supporting patients and families through significant health challenges while ensuring continuity of care.

Qualifications

The ideal candidate will possess the following:

• A Master’s degree in Nursing as a Nurse Practitioner with specialized training in adult care.
• Current New York State licenses as a Registered Professional Nurse and Nurse Practitioner.
• NP Board certification, ensuring adherence to the highest standards of nursing practice.
• Certifications in Basic Life Support (BCLS), Advanced Cardiovascular Life Support (ACLS), and required DEA and NPI certifications.
• Proficiency in electronic medical record systems and comfort with software like MS Word, PowerPoint, and Excel for documentation and communication.

Preferred Experience

Prospective candidates should have:

• A minimum of 2 years of experience in a hospital setting, ideally with a focus on cardiology.
• Experience managing advanced therapies for patients requiring outpatient intravenous support, inhaled medication therapy, or mechanical circulatory support.
• A proven track record of collaborative work within multidisciplinary teams, enhancing the patient experience and clinical outcomes.

Compensation and Benefits

We offer a competitive salary range between USD 120,000 and 220,000 annually, commensurate with the candidate's qualifications and experience. Our comprehensive benefits package includes:
